The form for your questions is at the bottom of this page. Here are a few reminder policies before you fill out the form:

In addition to ordering your reading I must have the Reading Request Form below completed and sent to me before I can actually do the reading for you.  The content of this form is emailed directly to my private email address. You can order your reading through the shopping cart and then fill out the form or fill out the form and then order the reading. Whatever is easiest for you.

With ALL of my readings, I do not set a time limit for the length of the reading, I do not set a word limit on my response and I do not charge per minute. I give readings in as much detail as possible and tell you everything that I’m seeing regardless of how long it takes to do it or how much I have to type. 

Enter any information that is pertinent to your reading(s) in the Questions and Additional Message (if you need more room) provided below. This would be where you would ask your specific questions, tell me your dream or psychic phenomena or include any other information that you would like to submit regarding the reading that you ordered. If you’re making payment via money order, check etc you can put it in this area as well instead of emailing me separately. And please remember I will not read someone else for you. I’ll only read for you.

In order to deliver your reading via email please enter your valid email address(es) in the space provided. Make sure your email address(es) is valid as you will still be charged if the email address doesn’t work and your reading has been completed.

By typing your full name in the electronic signature box you are agreeing that you are the person requesting the reading with Melissa Alvarez and you are verifying that you understand, are agreeing to, and will be legally bound by the terms and conditions of the DISCLAIMER, RELEASE & CONFIDENTIALITY READING REQUEST FORM detailed within this website and upon checkout of the shopping cart and that you are authorized by law to use the credit card that you will submit via PayPal for the purchase of a reading from Melissa Alvarez. The ELECTRONIC SIGNATURE must be your legal name including your first and last name. I can’t read for you if you only put your first name.
